Initial implementation strategy
- The browser and server are standard.
- Proxy and gateway use a MilliCent HTTP extension, w/payments piggy-backed on requests.
- Browser-proxy and gateway-server connections use HTTP basic auth.
- Proxy and browser may run on same machine.
- Gateway & server on one machine or inside firewall.
